They are different from nasolabial folds, which run from the side of the nose towards the corner of the mouth. Both can become more visible with age, but they sit in different parts of the face and are influenced by slightly different structures.

Why do marionette lines become deeper?

Loss of support through the mid-face

As the cheeks lose some of their youthful support, tissues can sit lower on the face. This can increase the weight and shadow around the lower face and make folds near the mouth look deeper.

Changes around the jaw and chin

Age-related changes to the jawbone and chin can alter the framework around the mouth. A small hollow can also develop beside the chin, which makes the line look more obvious.

Skin laxity and collagen loss

Thinner, less elastic skin folds more easily. Sun exposure and natural collagen decline can therefore make marionette lines more pronounced even if facial volume has not changed dramatically.

Facial movement

The muscles around the mouth are constantly active when we speak, smile, eat and express ourselves. In some people, the natural downward pull around the corners of the mouth contributes to the overall appearance.

Are marionette lines the same as jowls?

No, although they often appear together. Marionette lines sit beside the mouth and chin. Jowls affect the jawline. Both can be influenced by lower-face ageing, but each deserves its own assessment.

Can marionette lines be treated without filler?

Sometimes, yes. It depends on why the line is there.

If skin laxity is the main issue, a treatment aimed at collagen and firmness may be more relevant than adding volume. If the lower face has lost broader support, we may look at the cheeks, chin or jaw rather than treating the line in isolation.

What if the corners of the mouth turn down?

Some patients have a naturally downturned mouth even before deeper marionette lines develop. Muscle activity can contribute to this in selected cases, so the assessment may include how the lower-face muscles move as well as the static line you see at rest.

This does not mean everyone needs anti-wrinkle injections. It simply means that movement is one part of the picture and should be considered before choosing a treatment.

Why whole-face assessment matters

A deep line near the mouth may be caused partly by what is happening in the cheek, partly by the jawline and partly by the skin itself. Treating one line in isolation can therefore give a limited result.
